The herein named authority certifies that the bearer of this credential was enrolled in training and successfully completed performance testing in conformance with the standard(s) cited. This card must be available for inspection by any welding authority and may be revoked if welding ability or ethical representation are questioned. This card, and any documents thereby referred, shall be considered the property of the organization indicated above. Qualification will expire 6 months from the original test date unless continuity can be proven.
